MEDICATION MANAGEMENT TECHNOLOGY

Bridge Medical, Inc., an Amerisource Bergen company, headquartered in Solana Beach, California, is a technologies and services company, founded in 1996. Bridge offers proprietary software, and consulting services to help hospitals address the problem of medication, blood transfusion, and specimen tracking errors.

The MedPoint Medication Management System, first installed in a hospital in 1999, is focused at the bedside to verify the “five rights” (right patient, right medication/blood component, right dose/blood type, right route, and right time) prior to medication administration. The system provides a safety net at the point of care, when errors originating earlier in the process can be intercepted before harming the patient. In addition, the MedPoint system automates the medication administration record, making accurate documentation easier for nurses, and patient information more accessible to physicians, nurses, pharmacists, and phlebotomists. MedPoint also offers real-time, “near-miss” reporting to hospital quality improvement teams, so that systems flaws, the root cause of medication, blood transfusion, and specimen tracking error, can be identified and eliminated.

Bridge’s mission is to help hospitals reduce medication, blood transfusion, and specimen tracking errors, and continually improve the quality of the care they provide to patients. The MedPoint system has two overriding goals:

  • Protect the patient from harm of adverse drug, blood transfusion,
  • and specimen identification events.
  • Provide the hospital administration data that will empower the
  • proactive pursuit of a zero-tolerance error rate. In addition, MedPoint's key features are:
  • Five Rights Check
  • Beyond Five Rights Check warnings and messages
    • High-risk medications
    • Look alike / sound alike medications
    • Maximum dosing
    • Missed doses
    • Allergy checking
  • Clinical documentation / electronic MAR
  • Blood transfusion verification
  • Laboratory specimen identification and tracking
  • Medication error and near miss reporting
  • Clinical decision support.
